Patient Assessment: Conduct comprehensive assessments of residents to evaluate their physical abilities, mobility, and functional limitations.
Treatment Planning: Develop individualized treatment plans based on assessment findings, setting realistic and achievable goals in collaboration with the resident and the interdisciplinary care team.
Physical Therapy Interventions: Provide evidence-based physical therapy interventions, including therapeutic exercises, manual therapy, gait training, balance exercises, and other modalities as appropriate.
Mobility Aids and Assistive Devices: Recommend and educate residents on the proper use of mobility aids and assistive devices to improve their independence and safety.
Pain Management: Address pain management needs through therapeutic techniques and appropriate exercises to alleviate discomfort and enhance mobility.
Falls Prevention: Implement falls prevention strategies and exercises to reduce the risk of falls among residents.
Rehabilitation: Assist in the rehabilitation process for residents recovering from surgeries, injuries, or other medical conditions.
Patient Education: Educate residents and their caregivers on exercise routines, self-care techniques, and strategies to maintain functional abilities.
Progress Monitoring: Regularly assess and document residents' progress towards their therapy goals, modifying treatment plans as needed.
Interdisciplinary Collaboration: Work collaboratively with other healthcare professionals, including nurses, occupational therapists, and physicians, to provide comprehensive care to residents.
Safety Measures: Implement safety protocols during therapy sessions to ensure the well-being of residents and prevent injuries.
Documentation: Maintain accurate and up-to-date documentation of assessments, treatment plans, progress notes, and other relevant records.
